Management of Androgen Decline in Aging Men

چکیده

In both the scientific literature and lay press, new attentions recently have been focusing on low testosterone states in the aging male. This syndrome has been referred to as andropause, androgen deficiency in the aging male (ADAM), partial androgen deficiency in the aging male (PADAM), male climacteric, relative hypogonadism and hypoandrogenemia. There is neither agreement on the syndrome’s nomenclature nor consensus on its diagnosis. Although this evolving area of research is producing evidence for the validity of the syndrome and the benefits of its treatment, there are still many unanswered questions and controversies. This article reviews the epidemiology, diagnosis and treatment of androgen deficiency. For simplicity, this syndrome will be referred to as andropause throughout this review.

Interrelationship between Androgen Levels, Ageing, and Cognitive Functions

As men get older, there is a decline in functioning of many biological systems; the endocrine systems share such changes in hormone levels. Ageing in men is accompanied by progressive, but individually variable decline in serum testosterone production in healthy men especially in men over 60 years of age. Androgens determine the differentiation of male internal and external genitalia as well as...

Andropause: clinical implications of the decline in serum testosterone levels with aging in men.

T is now well accepted that serum testosterone (T) levels decline progressively with aging in men (1–5). This decline is associated with alterations in body composition; diminished energy, muscle strength, and physical function; reduced sexual function; depressed mood; and decreased cognitive function. Similar changes occur in young men with androgen deficiency and are improved with T replaceme...

The decline of androgen levels in elderly men and its clinical and therapeutic implications.

Aging in men is accompanied by a progressive, but individually variable decline of serum testosterone production, more than 20% of healthy men over 60 yr of age presenting with serum levels below the range for young men. Albeit the clinical picture of aging in men is reminiscent of that of hypogonadism in young men and decreased testosterone production appears to play a role in part of these cl...

Androgen replacement therapy in the aging male--a critical evaluation.

Recent years have seen an increasing interest in the study of the aging male, with a particular interest in the problem of whether so-called rejuvenating hormones and, more specifically, androgens can improve quality of life, counteract progressive skeletal muscle loss and strength, prevent falls and fractures, prolong independent living, and reduce the dependence on medical care. Almost a deca...

Androgen effects on skeletal muscle: implications for the development and management of frailty

Androgens have potent anabolic effects on skeletal muscle and decline with age in parallel to losses in muscle mass and strength. This loss of muscle mass and function, known as sarcopenia, is the central event in development of frailty, the vulnerable health status that presages adverse outcomes and rapid functional decline in older adults. The potential role of falling androgen levels in the ...
